1. How to use the tool?
  • Obtain the password giving access to the re-enrolment form in the communications sent or from Human Resources.
  • Click on the link "Add or Remove the Dental Care Benefit" at the bottom of this page.
  • Complete the form that appears on the page.
  • Use the "Submit my choice" button to confirm your choice.

2. Important Date
You must submit your choice by April 15, 2022 using the form on this site. If you do not wish to make a change to your current coverage, whether or not you are covered by the optional dental care benefit, you do not need to take any action.
